ABSTRACT

As armas de fogo são instrumentos letais que estão relacionados a uma grande quantia de homicídios no Brasil, além de traumas e violências. Assim, o atendimento pré-hospitalar e hospitalar é importante na tentativa de diminuir os índices de mortalidade por causas externas. Esta revisão de literatura teve como objetivo demonstrar as formas protocoladas de atendimento atuais a um paciente com trauma por projétil de arma de fogo (AU).


Firearms are lethal instruments that are related to a large amount of homicides in Brazil, as well as traumas and violence. Thus, pre-hospital and hospital care is important in the attempt to reduce mortality rates due to external causes. This literature review aimed to demonstrate the current protocol forms of care for a patient with gunshot wound. (AU)

ABSTRACT

Resumo As fístulas arteriovenosas (FAVs) traumáticas envolvendo os vasos axilares e subclávios são incomuns e correspondem de 5 a 10% de todos os traumas arteriais. A anatomia complexa dessa região torna desafiador o tratamento desse segmento. Neste desafio terapêutico, descrevemos o caso de um homem de 73 anos, encaminhado por edema progressivo e úlcera no membro superior direito, com história pregressa de ferimento por arma de fogo na região infraclavicular direita há cerca de 50 anos. Foi realizada angiotomografia e identificou-se FAV axilo-axilar associada à tortuosidade e dilatação aneurismática de artéria subclávia a jusante. O paciente foi submetido à intervenção endovascular com endoprótese cônica (monoilíaca) 26 × 14 × 90 mm Braile® na artéria subclávia aneurismática, posterior à saída da artéria vertebral direita, e endoprótese monoilíaca 16 × 16 × 95 mm Excluder® com sobreposição na primeira prótese, apresentando resultado satisfatório. Portanto, descreve-se a possibilidade de utilização de endoprótese aórtica em situação incomum e de exceção, com sucesso.


Abstract Traumatic arteriovenous fistulas (AVFs) involving the axillary and subclavian vessels are uncommon and account for 5 to 10% of all arterial traumas. The complex anatomy of this region makes treatment of this segment challenging. In this therapeutic challenge, we describe the case of a 73-year-old man, referred for progressive edema and ulceration involving the right upper limb and with a history of gunshot wound to the right infraclavicular region about 50 years previously. Angiotomography was performed and an axillary-axillary AVF was found, associated with tortuosity and aneurysmatic dilation of the subclavian artery downstream. He underwent endovascular intervention and a conical (monoiliac) 26 × 14 × 90 mm Braile® endoprosthesis was used in the aneurysmatic subclavian artery, posterior to the exit of the right vertebral artery and a 16 × 16 × 95mm Excluder® monoiliac endoprosthesis was placed overlapping the first prosthesis, showing a satisfactory result. Therefore, the possibility of successfully using aortic endoprostheses in an unusual and exceptional situation is described.

Resumen El plomo es un metal que encontramos depositado en el ambiente a causa de la contaminación o la explotación mineral. Debido a que este componente ha tenido una gran diversidad de usos en su historia, los medios y formas de intoxicación ha generado un cambio en la perspectiva de los componentes que pueden generar una intoxicación por plomo. Durante la investigación de la intoxicación por plomo mediante proyectiles de arma de fuego se analizó la posibilidad de que se produzca intoxicación por plomo en un caso de una femenina con retención del proyectil por arma de fuego en hueso occipital con más de 50 años, abordando desde una revisión bibliográfica, la historia de formas o productos en que este metal producía intoxicación, los mecanismos de absorción, distribución y metabolismo en el cuerpo, el mecanismo de acción, el daño a los órganos, los hallazgos clínicos, la composición de la bala y su relación médico legal para confirmar la presencia de un proyectil por arma de fuego retenido en el cuerpo con la finalidad de descartar si es posible una intoxicación por plomo por un proyectil de arma de fuego retenido en hueso occipital.


Abstract Lead is a metal that we find deposited in the environment because of pollution or mineral exploration. In reason that this component had a great diversity of uses in its history, the means and forms of intoxication have generated a change in the perspective of which components could generate a lead poisoning. During the investigation of lead poisoning through firearm projectile it was analyzed the possibility that a lead poison gets produced in a feminine that holds a firearm projectile for more than 50 years in the occipital bone, addressing from a bibliographic review, the history of forms or products in which this metal produced intoxication, the mechanisms of absorption, distribution and metabolism in the body, the mechanism of action, damage to the organs, the clinical findings, the composition of the bullet and its legal medical relationship to confirm the presence of a projectile by firearm retained in the body in order to rule out if lead poisoning is possible by a firearm projectile retained in occipital bone.

Resumen Introducción: Los traumatismos torácicos por armas de fuego (TTAF) son cada vez más frecuentes. Objetivos: Describir características clínicas, morbilidad, mortalidad y la evolución a través del tiempo de hospitalizados por TTAF. Materiales y Método: Estudio analítico longitudinal. Período enero de 1981-diciembre de 2018. Revisión base de datos, protocolos prospectivos y fichas clínicas. Se utilizó planilla Microsoft Excel® y programa SPSS24® con chi cuadrado y de Mann-Whitney. Descripción de características de TTAF en pacientes hospitalizados y comparación por períodos. Se calcularon índices de gravedad del traumatismo (IGT): Injury Severity Score (ISS), Revised Trauma Score (RTS-T), Trauma Injury Severity Score (TRISS). Resultados: Total 4.306 pacientes hospitalizados por traumatismo torácico (TT), 205 (4,8%) hospitalizados por TTAF. Hombres: 188 (91,7%), edad promedio 28,8 ± 11,2 años, TTAF aislado 115 (56,1%), asociado a lesiones extratorácicas 90 (43,9%), de estos 55 (26,8%) se consideraron politraumatismos. Mecanismo: Agresión 193 (94,1%), autoagresión 11 (5,4%) y accidental 1 (0,5%). Lesiones y/o hallazgos torácicos más frecuentes: Hemotórax 127 (62,0%), neumotórax 96 (46,8%) y contusión pulmonar 51 (24,9%). Tratamiento definitivo: Pleurotomía 88 (42,9%), cirugía 71 (34,6%) y tratamiento médico 46 (22,4%). Mediana de hospitalización 7 días. Según IGT: ISS promedio 16,7 ± 11,7, RTS-T promedio 11,1 ± 2,1, TRISS promedio 9,6. Morbilidad: 44 (21,5%). Mortalidad: 14 (6,8%). En los diferentes períodos, se observó aumento de politraumatismos y TRISS, sin cambios en mortalidad. Discusión: La mayoría de los TTAF fueron aislados. Aproximadamente un tercio de los pacientes requirió cirugía. La mortalidad observada es menor a la esperada. Se observan cambios en los TTAF a través del tiempo.


Background: Thoracic trauma by firearms (TTF) are increasingly frequent. Aim: To describe clinical characteristics, morbidity and mortality and the evolution over time of patients hospitalized due TTF. Materials and Method: Longitudinal analytical study. Period January 1981 - December 2018. Database review, prospective protocols and clinical files. Microsoft Excel® spreadsheet and SPSS24® program with chi square and Mann-Whitney tests were used. Description of characteristics of TTF in hospitalized patients and comparison of TTF by periods. Trauma severity indexes (TSI) were calculated: Injury Severity Score (ISS), Revised Trauma Score (RTS-T), Trauma Injury Severity Score (TRISS). Results: Total: 4306 hospitalized patients due thoracic trauma (TT), 205 (4.8%) hospitalized due TTF. Men: 188 (91.7%), average age 28.8 ± 11.2 years, isolated TTF 115 (56.1%), associated with extrathoracic lesions 90 (43.9%), and of these 55 (26.8%) were considered polytraumatism. Mechanisms: aggression 193 (94.1%), self-harm 11 (5.4%) and accidental 1 (0.5%). Frequent thoracic injuries and/or findings: hemothorax 127 (62.0%), pneumothorax 96 (46.8%) and pulmonary contusion 51 (24.9%). Definitive treatment: Pleurotomy 88 (42.9%), surgery 71 (34.6%) and medical treatment 46 (22.4%). Median hospitalization 7 days. According TSI: Average ISS 16.7 ± 11.7, average RTS-T 11.1 ± 2.1, average TRISS 9.6. Morbidity: 44 (21.5%). Mortality: 14 (6.8%). There is an increase in polytraumatism and average TRISS, without changes in mortality. Discussion: The majority of TTF were isolated TT. Approximately one third of patients required surgery. The observed mortality is lower than expected. Changes in TTF were observed over time.

ANTECEDENTES La ubicación precisa de un cuerpo extraño intraocular es crucial para el manejo de pacientes con trauma ocular abierto. La tomografía computarizada se usa habitualmente para detectar su ubicación en el segmento posterior. Reportamos tres casos con diferentes cuerpos extraños intraoculares en el segmento posterior, que fueron localizados con precisión mediante la tomografía computarizada y ecografía modo B. PRESENTACIÖN DEL CASO Presentamos tres casos con diferentes mecanismos de trauma, tipos de cuerpo extraño intraocular, síntomas clínicos y pronóstico visual. La tomografía computarizada determinó la ubicación exacta de todos los cuerpos extraños intraoculares en el segmento posterior. El ultrasonido modo B se realizó en un paciente con un cuerpo extraño intraocular no metálico. El primer caso tuvo una lesión ocular perforante con un cuerpo extraño intraorbitario; el caso dos y el caso tres presentaron diferentes tipos de cuerpos extraños intraoculares con pronóstico diferente. El manejo y el pronóstico fue distinto en todos los casos; todos se manejaron con éxito. La ubicación exacta de los cuerpos extraños intraoculares utilizando las diferentes modalidades de diagnóstico es importante en estos pacientes. Estos casos sirven como recordatorio de que el uso adecuado de las pruebas de imagen es indispensable en el contexto de un cuerpo extraño intraocular relacionado con trauma ocular abierto. CONCLUSIÓN Las diferentes técnicas de imágenes son muy importantes para la detección de un cuerpo extraño intraocular. La tomografía computarizada es una de las modalidades de imagen más simple y efectiva para la localización de cuerpos extraños intraoculares relacionadas con trauma ocular abierto.


BACKGROUND: Determining the precise location of intraocular foreign bodies is crucial for the management of patients with open-globe injury. Computed tomography is the most common method for detecting intraocular foreign bodies in the posterior segment. In this article, we describe three cases of open-globe injury with different types of intraocular foreign bodies in the posterior segment that were accurately located using computed tomography scans and B-scan ultrasonography. CASE PRESENTATION: Each of the three cases of open-globe injury described in this report had different types of ocular trauma, clinical symptoms, and intraocular foreign bodies. Computed tomography scans showed the exact location of the intraocular foreign bodies in the posterior segment in two of the three cases. A B-scan ultrasound was used to determine the location of a non-metallic intraocular foreign body in the third case. All three patients had intraocular foreign bodies, and one of them had an additional orbital foreign body. Case 1 had a perforating eye injury with the additional intraorbital foreign body; Cases 2 and Case 3 had different types of intraocular foreign bodies and prognoses. Various treatment approaches were used, ranging from observation to surgery, depending on the location of the intraocular foreign bodies, and all cases were successfully managed. These three cases show that proper use of various types of imaging tests is indispensable in the context of an intraocular foreign body related to open-globe injury. CONCLUSION: Imaging techniques are crucial for the detection of an intraocular foreign body, and computed tomography is one of the simplest and most useful, especially in cases of open-globe injury.

Resumen: Antecedentes: La introducción de armas de fuego ha incrementado las muertes, asesinatos y suicidios. Las lesiones por proyectil de arma de fuego (PAF) afectan cada vez más a poblaciones civiles (500,000 lesiones anualmente) y es la segunda causa de muerte en jóvenes. La mitad requiere tratamiento de fracturas (45%), acompañado de reportes de 6% que desarrollaron una infección profunda. Material y métodos: Estudio retrospectivo, observacional y transversal en cuatro hospitales de la Secretaría de Salud de la CDMX de la frecuencia de infección en fracturas expuestas por PAF y si existe algún otro factor que acompañe a la infección. Resultados: Se encontraron en la revisión de los expedientes de los cuatro hospitales a 67 pacientes, hombres, con una incidencia de infecciones de 4.5%, al analizar los factores de riesgo éstos no presentaron asociación significativa. Discusión: Ningún factor de riesgo posee significancia estadística de que se presente algún proceso infeccioso, probablemente por un buen tratamiento inicial.


Abstract: Background: The introduction of firearms has increased deaths, assassinations and suicides. Firearm projectile injuries (PAF) increasingly affect civilian populations (500,000 injuries annually), it is the second cause of death in young people. Half required fracture treatment (45%), accompanied by reports of 6% who developed a deep infection. Material and methods: Retrospective, observational, cross-sectional study in four hospitals of the Secretary of Health of CDMX of the frequency of infection in fractures exposed by PAF and if there is any other factor that is associated with the infection. Results: In the review of the files of the four hospitals, 67 patients were found, men, with an incidence of infections of 4.5%, when analyzing the risk factors these did not present a significant association. Discussion: No risk factor has statistical significance for any infectious process to occur, probably due to a good initial treatment.

policial masculino de 47 anos foi atendido em consultório com queixa de dispneia aos esforços, edema e dor importantes em braço direito. Relatou ferimento por arma de fogo infraclavicular direito 7 meses antes. Tomografia de tórax mostrou grande dilatação de veia subclávia, veias cervicais e de membro superior direito sem identificação da comunicação arteriovenosa. O paciente foi internado antes da data prevista para tratamento por piora clínica e foi submetido a implante de stent revestido Fluency 8x100 mm em artéria subclávia direita por técnica do varal. Angiografia de controle mostrou artéria subclávia pérvia e fechamento da fístula. Houve melhora dos sintomas em braço direito no primeiro dia após o procedimento. As lesões traumáticas da artéria subclávia são incomuns, porém podem evoluir com alto índice de morbimortalidade O trauma penetrante é o principal agente etiológico, e fístulas arteriovenosas devem ser pesquisadas durante o atendimento do paciente com lesões penetrantes em trajeto vascular


A 47-year-old male police officer presented at an outpatients consulting room complaining of exertional dyspnea and swelling and pain in the right arm. He had suffered a perforating gunshot wound of the right infraclavicular region 7 months previously. A chest tomography showed considerable dilatation of the subclavian and cervical veins and veins of the right upper limb, with no clear point of arteriovenous communication. His symptoms exacerbated and he was admitted to hospital before the date scheduled for treatment. He underwent endovascular treatment with an 8x100 mm Fluency covered stent that was placed in the right subclavian artery using the through-and-through technique. Control angiography showed that the fistulous defect had been completely sealed. There was significant relief of the symptoms on the first day after the operation. Traumatic lesions of the subclavian artery are rare, but can be associated with high morbidity and mortality rates. Penetrating trauma is the main cause and arteriovenous fistulas should be ruled out when evaluating penetrating injuries in vascular territorie

Resumo Um traumatismo ocular contuso pode gerar uma vasta gama de lesões intraoculares que são dependentes da quantidade de energia que o objeto transfere ao olho durante o impacto. As lesões da íris, malha trabecular e corpo ciliar podem repercutir na drenagem do humor aquoso. A ocorrência de uma ciclodiálise caracterizada pelo descolamento do corpo ciliar da parede escleral, criando uma comunicação entre a câmara anterior e o espaço supracoroidiano, pode provocar importante redução da pressão intraocular. Esta diminuição pressórica é a responsável pela ocorrência da maculopatia hipotônica que pode cursar com diminuição irreversível da acuidade visual. Não existe consenso na literatura sobre o tratamento das ciclodiálises. Neste relato, descrevemos um caso de ciclodiálise traumática pós impacto com projétil de arma de fogo tratada com aplicação de laser de argônio.


Abstract Blunt eye trauma can generate a wide range of intraocular lesions that are dependent on the amount of energy the object transfers to the eye during the impact. Lesions of the iris, trabecular meshwork and ciliary body may influence aqueous humor drainage. The cyclodialysis which is characterized by the detachment of ciliary body from the scleral wall, creating a communication between the anterior chamber to supracoroid space, can cause a several reduction of intraocular pressure. This pressure decrease is responsible for the occurrence of hypotonic maculopathy which may induce a severe impact on visual acuity. There is no consensus in the literature about cyclodialysis treatment. In this report, we describe a case of traumatic cyclodialysis after impact with a firearm projectile treated with argon laser photocoagulation.

ABSTRACT

ABSTRACT A 44-year-old man was referred for evaluation of pain and temporal floaters after receiving a rebounded bullet impact to his right eye. Typical funduscopic findings, together with the confirmed presence of an intraorbital metallic foreign body, led to the diagnosis of chorioretinitis sclopetaria. Conservative management was performed as no severe symptoms were observed. The favorable clinical outcome was confirmed in subsequent reviews. Chorioretinitis sclopetaria is characterized by a proliferative chorioretinal inflammation as a consequence of the expansive wave caused by the entrance of a bullet between the eyeball and the orbit.


RESUMO Um homem de 44 anos foi encaminhado para avaliação de dor e flutuadores temporais após receber um impacto de bala ressaltado em seu olho direito. Achados fundoscópicos típicos, juntamente com a presença confirmada de um corpo estranho metálico intraorbitário, levaram ao diagnóstico de coriorretinite esclopetária. O manejo conservador foi realizado, pois não foram observados sintomas graves. O desfecho clínico favorável foi confirmado em revisões subsequentes. A coriorretinite esclopetária é caracterizada por uma inflamação coriorretiniana proliferativa como consequência da onda expansiva causada pela entrada de uma bala entre o globo ocular e a órbita.

Abstract The surgical management of firearm injuries to the zygomaticomaxillary complex is a topic that needs to be addressed with much care. The complications that can arise during treatment, as well as the potential residual problems that can appear, lead to the need of recognizing these injuries as a maxillofacial treatment challenge. The infraorbital zone is the second most common area of injury in the face, accounting for up to 45% of midfacial fractures. The case presented is of a 54 year old man affected by the backward firing of a gun rifle that caused an oblique impaction towards the posterior lateral inferior side of the infraorbital complex. He was rapidly stabilized with an antibiotic and analgesic scheme, yet presented the following: a comminute fracture of the orbit floor with consequent communication towards the maxillary sinus, a non-displacing fracture of the frontomalar suture, a non-displacing fracture of the zygomatic arch and a lesion with loss of osseous tissue in the petrous region of the temporal bone. Rigid fixation followed stabilization, and the area was covered with a titanium mesh. Occasional tinnitus and photopsia were reported by the patient upon return, yet all other evaluated factors were in order. There have been different forms of management described in the literature, and it is recognized that each patient has different needs, therefore each case must be closely evaluated, so it is approached in the most appropriate manner.


Resumen El manejo quirúrgico de las lesiones por armas de fuego al complejo cigomaticomaxilar representa una situación compleja en la práctica maxilofacial que debe ser abordada con extrema cautela, debido a las complicaciones que pueden surgir durante el tratamiento y los potenciales problemas residuales. La zona infraorbitaria es la segunda área más común de lesiones en la cara, representando hasta 45% de las fracturas mediocraneales. El presente caso es de un masculino de 54 años de edad, que sufrió trauma ocasionado por la parte trasera de un rifle de caza que causó un impacto en sentido oblicuo hacia el lado lateral inferior del complejo infraorbitario. El paciente fue estabilizado con esquema antibiótico y analgésico, el diagnóstico fue: fractura cominuta del piso de la órbita, con consecuente comunicación al seno maxilar, fractura no desplazada de la sutura frontomalar, fractura no desplazada del arco cigomático, y lesión con pérdida de tejido óseo en la región petrosa del hueso temporal. El tratamiento quirúrgico consistió en reducción de las fracturas y fijación rígida de las estructuras. Al control post operatorio el paciente refirió acufenos y fotopsia ocasional, no presentando otras alteraciones. Se ha descrito en la literatura diferentes abordajes para el manejo de estos traumatismos, sin embargo cada caso debe ser individualizado y evaluado de forma minuciosa para que éste, sea el adecuado.

ABSTRACT

Abstract Embolization by a dislodged projectile is a rare complication that may occur in cases of gunshot cardiac injuries. We report a case of a firearm projectile cardiac injury that evolved, with dislocation of the projectile during cardiac surgery, into embolization of the right external carotid artery.

ABSTRACT

A perda de grande parte do osso mandibular, seja devido à necessidade de ressecção óssea ou por outros fatores como a ocorrência de traumatismos, afeta diretamente a vida dos pacientes. Mesmo com a melhoria contínua dos materiais, surgimento de novas tecnologias e técnicas cirúrgicas, o tratamento dessas grandes lesões e perdas ósseas permanece como grande desafio ao cirurgião. Nesse sentido, o objetivo do presente trabalho foi relatar dois casos clínicos de pacientes, sendo um de fibroma ossificante central de mandíbula que foi submetido à ressecção mandibular e tratado através da colocação de uma placa de reconstrução do sistema 2.4 mm. O segundo caso envolveu perda de substancia devido a paciente ter sido vitima de ferimento por arma de fogo (FAF), evoluindo com fratura do sistema de fixação colocado na primeira cirurgia. Em segundo momento cirúrgico a paciente foi submetida à reconstrução óssea do defeito, associado à colocação de nova fixação. Deste modo, podemos afirmar que os procedimentos reconstrutivos são importantes instrumentos para tratamento dos pacientes.


The loss of much of the mandibular bone, is due to the need for bone resection or by other factors such as the occurrence of injuries, directly affects the lives of patients. Even with the continuous improvement of materials, development of new technologies and surgical techniques, treatment of these large lesions and bone loss is still a major challenge for the surgeon. In this sense, the objective of this study was to report two clinical cases of patients, a central ossifying fibroma of the mandible who underwent mandibular resection and treated by placement of a reconstruction plate system 2.4mm. The second case involved loss of substance because the patient has been the victim of injury by firearms (FAF), evolving with fracture fixation system placed in the first surgery. In the second surgical time the patient underwent bone reconstruction of the defect associated with the placement of new fixation. Thus, we can say that reconstructive procedures are important tools for treatment of patients.

ABSTRACT

The aim of this study was to analyze and compare the different lines of surgical management and associated complications of extraperitoneal rectal injuries sustained in Aleppo through Syrian war to get the best one of them. This is a prospective study. Between July 2012 and July 2014, 31 patients with gunshot injuries to the extraperitoneal rectum were treated at the Aleppo University Hospital. Patients with only intraperitoneal rectal injuries were not included in the study. The surgical management of rectal injuries was evaluated, specifically looking at the utilization of proximal diversion, distal washout, and presacral drainage. Complications were compared between the different lines of surgical management. Thirty one patients who sustained penetrating rectal injuries were included in this study. Surgical management included diversion and presacral drainage in 17 patients [55%], diversion alone in 9 patients [29%], and diversion, distal washout and presacral drainage in 5 patients [16%]. Complications were identified in 8 patients [26%]. There were two deaths in the study group. In this cohort most cases have been successfully managed by proximal diversion and presacral drainage. We have not seen any additional benefit when distal washout was performed; and we have not closed any extraperitoneal rectal injury because it is unnecessary, very difficult, and may be impossible or cause additional injuries

ABSTRACT

Objetivo O presente trabalho objetivou descrever os aspectos clínicos e epidemiológicos do traumatismo raquimedular causado por ferimentos de armas de fogo em pacientes internados no Hospital Metropolitano de Urgências e Emergências, em Belém/PA, no período de janeiro de 2008 a dezembro de 2013. Método Estudo transversal, retrospectivo e descritivo, com base na análise de prontuários de 89 pacientes vítimas de trauma raquimedular por ferimento por arma de fogo, de acordo com os dados obtidos no setor de registro hospitalar. Resultados Os traumas decorrentes de projéteis de armas de fogo foram responsá- veis por 9,2% dos traumas raquimedulares, apresentando uma média de 14,8 novos casos por ano; 86,5% destes eram pacientes do sexo masculino, 73,1% apresentavam menos de 30 anos. O segmento cervical foi o mais acometido (46%) e 51,8% dos pacientes foram classificados como ASIA A. A maior parte foi tratada conservadoramente (75,3%) e 56,2% dos pacientes evoluíram com alguma complicação durante a internação, sendo mais prevalentes as complicações infecciosas (45%). A taxa de óbito foi de 18%. Conclusão O trauma raquimedular decorrente de ferimentos de armas de fogo nos pacientes atendidos no Hospital Metropolitano de Urgências e Emergências acomete principalmente homens jovens. O nível cervical foi o mais acometido e a classificação ASIA A foi a mais prevalente. O tratamento conservador foi adotado na maioria dos casos, sendo que mais da metade dos pacientes evoluiu com complicações, especialmente infecciosas, enquanto uma significativa parcela evoluiu para óbito.


Objective This study aimed to describe the clinical and epidemiological aspects of Spinal Cord Injury caused by firearms in patients admitted to the Metropolitan Urgency and Emergency Hospital, Belém, Pará, from January 2008 to December 2013.Methods It was developed a cross-sectional, retrospective and descriptive study, based on analysis of medical records of 89 patients diagnosed with Spinal Cord Injury by firearm's injury. Results It was observed that firearm's injury accounted for 9,2% of Spinal Cord Injury, with a average of 14,8 news cases per year; 86,5% of these were male; 73,1% had less than thirty years. The cervical segment was the most affected (46%) and 51,8% of the pacients were classified as ASIA A. The most were treated conservatively (75,3%) and 56,2% had some complication during the hospitalization, the infectious complications were the more frequent (45%). The dead rate was 18%. Conclusion Thus, this study concluded that the SCI by firearm's injury in patients treated at Metropolitan Urgency and Emergency Hospital affects mainly young men, the traumas occurred mainly in the cervical segment and the classification ASIA A was the most prevalent. Conservative treatment was adopted in most cases and more than half of the patients developed complications, especially infectious, while a significant portion evolve to death.

ABSTRACT

The authors describe treatment of a patient who presented an arteriovenous fistula between the popliteal vessels more than 20 years after a gunshot wound. The patient underwent endovascular treatment using Viabahn (Gore®) stent grafts, but, because of the large disparity in popliteal artery diameters proximal and distal of the fistula, the endovascular treatment was unsuccessful. The superficial femoral artery was then banded around the stent graft that was deployed previously. This improvised strategy allowed an open surgical approach to be performed far from the fistula site, reducing the risks of operating in a region with anatomic distortions and significant enlargement of the surrounding venous structures, which would certainly have increased the likelihood of iatrogenic injuries.


Os autores descrevem o tratamento de um paciente portador de fístula arteriovenosa entre vasos poplíteos com mais de 20 anos de evolução, após ferimento por projétil de arma de fogo. O paciente foi submetido a tratamento endovascular pelo implante de stents revestidos Viabahn (Gore®), porém o mesmo não foi bem sucedido devido à diferença de diâmetro entre a artéria poplítea proximal e a distal à fístula. A artéria femoral superficial foi submetida à cerclagem com fios de sutura ao redor do stentgraft previamente implantado. Esse recurso de improvisação permitiu uma abordagem distante do sítio da fístula, diminuindo os riscos do acesso cirúrgico em uma região com distorções anatômicas e vasos sanguíneos ectasiados, o que certamente acarretaria maior risco de lesões iatrogênicas.

O nervo facial, quando seccionado, pode ser reparado em seu tronco por enxerto ou anastomose. OBJETIVO: Discutir as técnicas em questão e mostrar o que podemos esperar das mesmas. MÉTODO: Foram operados sete pacientes com secção do nervo facial. As cirurgias foram quatro enxertos e três anastomoses. Cinco paralisias foram iatrogênicas e duas por projéteis de arma de fogo. A avaliação da recuperação motora foi feita pela tabela de Janssen. RESULTADOS: Obtivemos 72,5% em média de recuperação motora nos casos de enxerto e 73,3% nos casos de anastomose. CONCLUSÃO: 1. Enxertos e anastomoses são soluções adequadas para reparar o nervo facial seccionado, mas nunca permitem uma recuperação facial completa, podendo ocorrer sincinesias. 2. Em princípio, o nosso objetivo é fazer a anastomose, mas quando existe tração mínima nos cotos do facial, preferimos os enxertos. 3. Em ambas as técnicas, conseguimos acima de 70% de recuperação motora em média (72,5% nos enxertos e 73,3% nas anastomoses).


Sectioned facial nerves can be repaired with grafting or end-to-end anastomosis. OBJECTIVE: To discuss these repair procedures and what can be expected of them. METHOD: Seven patients with sectioned facial nerves were included in the study. Four underwent grafting and three were offered end-to-end anastomosis. Facial nerve palsy was iatrogenic in five patients and was caused by bullet wounds in two. Assessment of motor function recovery was based on Janssen's scale. RESULTS: Mean motor recovery was rated at 72.5% for subjects offered grafting and 73.3% for patients submitted to anastomosis. CONCLUSION: 1. Grafting and anastomosis are proper solutions to repair sectioned facial nerves; complete recovery is never attained; synkinesis may occur. 2. In principle anastomosis is the procedure of choice, but when there is minimal traction in the facial nerve stump grafting is preferred. 3. Both procedures yielded mean motor recovery rates above 70% (72.5% for grafting and 73.3% for anastomosis).

En 2011, hubo 8,7 millones de nuevos casos de tuberculosis activa en todo el mundo, causando alrededor de 1,7 millones de muertes cada año; el número de nuevos casos (más de 9 millones) es mayor que en cualquier otro momento de la historia a nivel mundial. En muchas regiones en las cuales la tuberculosis es altamente endémica, el diagnóstico sigue basándose en la antigua microscopía de esputo. La determinación de (Adenosin Deaminasa) ADA en líquidos biológicos, especialmente en líquido pleural, ha resultado ser altamente sensible para el diagnóstico de Tuberculosis, con el beneficio, además, de ser una prueba rápida y económica. Presentamos el caso de un paciente masculino de 34 años de edad, con historia de abuso de ilícitos inhalados, hemicolectomia por herida de arma de fuego + colostomía y contacto intradomiciliario de tuberculosis. Cursa con caquexia, astenia, hiporexia, fiebre, tos y disnea. Durante la hospitalización se encontró anemia, trombocitopenia, derrame pleural tipo exudado, mononucleares, ADA negativo, lesiones pulmonares nodulares, y evidencia por estudios de imagen, de afectación ganglionar sistémica y ósea expresados como lesiones osteolíticas por TC y posteriormente con hiperactividad metabólica de la 8FDG por CT-PET. El diagnóstico microbiológico se obtuvo después de 8 semanas de hospitalización y fue infección por Micobacterium tuberculosis.


In 2011, there were 8.7 million new cases of active TB worldwide. this causes about 1.7 million deaths each year and the number of new cases (more than 9 million) is greater than at any other time. In many regions where TB is highly endemic, the diagnosis is still based on the old sputum microscopy. The (Adenosin Deaminase) ADA determination in biological fluids, especially in pleural fluid has proved to be highly sensitive for the diagnosis of tuberculosis, and has the benefit of being a quick and inexpensive test.We report the case of a 34 year-old male patient with abuse of illicit inhaled drugs, a hemicolectomy+ colostomy caused by an old gunshot wound and contact with intradomicilliary TB. He consulted because of cachexia, asthenia, hyporexia, fever, cough and dyspnea. During his hospitalization progressive pancytopenia appeared , as well as exudative pleural effusion with mononuclear cells, a negative ADA, nodular lung lesions, and images of lymph nodes and many bone osteolytic lesions seen by CT. Later metabolic hyperactivity 8FDG by CT-PET was found. The microbiological diagnosisby culture was Mycobacterium tuberculosis after 8 weeks of hospitalization.

JUSTICATIVA E OBJETIVOS: O saturnismo decorre da intoxicação crônica pelo chumbo, comum após exposição ocupacional. Projéteis retidos no corpo podem resultar em elevados níveis plasmáticos de chumbo com inúmeras repercussões clínicas, principalmente neurológicas. Apresentamos um caso de paciente sintomático, sem diagnóstico, que foi detectado durante visita pré-anestésica. RELATO DO CASO: Paciente apresentado para retirada de projétil alojado em joelho esquerdo há 14 anos, com polineuropatia sem investigação. Durante visita pré-anestésica foi levantada a hipótese de saturnismo, confirmada pelos altos níveis plasmáticos de chumbo (129 µg.dL-1). A cirurgia foi realizada sob anestesia geral antes do início da quelação do chumbo. Durante indução anestésica, o paciente evoluiu com apneia, sem rigidez torácica após 50 µg de fentanil. CONCLUSÃO: A intoxicação por chumbo interfere com a função de diversos órgãos e sistemas, altera o metabolismo oxidativo e os mecanismos intracelulares de sinalização. O acometimento neurológico, com redução da condução neural merece especial atenção do anestesiologista.


BACKGROUND AND OBJECTIVES: Lead poisoning, also Saturnism, results from chronic lead poisoning, common after occupational exposure. Projectiles retained in the body may result in increased levels of plasma lead with numerous clinical implications, particularly neurological. We report a case of a symptomatic patient, undiagnosed, whose diagnosis was made during the preanesthesia evaluation. CASE REPORT: The patient was attended at our hospital to remove a bullet lodged in his left knee 14 years ago, presenting with polyneuropathy without investigation. During the preanesthetic visit, saturnism was hypothesized and confirmed by high plasma lead levels (129 µg.dL-1). Surgery was performed under general anesthesia before starting lead chelation. During anesthesia, the patient developed apnea, without chest-wall rigidity after administration of fentanyl (50 mg). CONCLUSION: Lead poisoning interferes with the function of various organs and systems, alters oxidative metabolism and intracellular signaling mechanisms. Neurological involvement, with reduced neural conduction, deserves special attention from anesthesiologists.


JUSTIFICATIVA Y OBJETIVOS: El saturnismo proviene de la intoxicación crónica por el plomo común después de la exposición ocupacional. Los proyectiles que el cuerpo retiene pueden traer como resultado elevados niveles plasmáticos de plomo con innumerables repercusiones clínicas, principalmente neurológicas. Presentamos aquí un caso de paciente sintomático, sin diagnóstico, y que fue detectado durante la visita preanestésica. RELATO DEL CASO: Paciente que se presentó para la retirada de una bala que se había alojado en la rodilla izquierda hacía 14 años, con polineuropatía sin investigación. Durante la visita preanestésica se pensó en saturnismo, lo que fue confirmado por los altos niveles plasmáticos de plomo (129 µg.dL-1). La cirugía fue realizada bajo anestesia general antes del inicio de la quelación del plomo. Durante la inducción anestésica, el paciente evolucionó con apnea sin rigidez torácica después de 50 µg de fentanilo. CONCLUSIONES: La intoxicación por plomo interfiere con la función de diversos órganos y sistemas, altera el metabolismo oxidativo y los mecanismos intracelulares de señalización. El compromiso neurológico, con la reducción de la conducción neural, merece una especial atención por parte del anestesiólogo.
